The Welsh Home's admission team works hard to coordinate a smooth transition from the hospital to skilled nursing care. Once a resident arrives, our interdisciplinary team of doctors, therapists, nurses, state tested nursing assistants, social workers, life enrichment professionals, the family and resident work together to outline the goals and determine a plan of care to meet those goals. Our discharge planner starts working from the time of admission to coordinate a seamless transition home. Our Medicare certified therapy services are available seven days a week.
